We welcome new patients to the practice. Please ask for the appropriate registration forms at Reception or print them off from our website. Please complete the Registration Form, GMS1 Health Questionnaire Form and the Identification and Ethnic Origin Form.
There are now two projects running side by side by the Government with regards to your patient data – Summary Care Record (SCR) and the Care.data (HSCIC). You have a choice on whether you want to share your data. The SCR is to be used in circumstances where another health care professional needs to access some of your medical records. They can view your allergies/adverse reactions, current medication and active problems. For more information please see http://www.nhscarerecords.nhs.uk The Care.data (HSCIC) is more of a research project. Please read the HSIC leaflet and if you require further information go to http://www.nhs.uk/NHSEngland/thenhs/records/healthrecords/Pages/care-data.aspx . If required complete the Summary Care Record and Care.Data Overview Form and return completed form to our reception.
Please ensure that all the relevant forms are completed and that you complete all the sections on the forms including your NHS number if known and full details of your previous doctor. This will enable us to retrieve your medical notes more quickly. Please bring all completed forms to our reception where the reception staff will be happy to answer any queries you may have. Your will need to complete the registration forms for each member of the family that wishes to register with our practice. You may register with any GP in the practice and you can see whichever GP you so choose. The practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.

People who live in the inner boundary area (red line) can register with the practice. If any currently registered patients move into the outer boundary area (green line) they can still retain their registration.
